I acknowledge by signing this agreement that I have been given the full opportunity to ask any and all questions which I might have about the obtaining of a tattoo and that all of my questions have been answered to my full satisfaction. I specifically acknowledge I have been advised of the facts and matters set forth below and I agree as follows:

I am of legal age (18 years or older) or have the legal guardian's consent.

I have discussed the tattoo design, size, placement, and color with the tattoo artist.

I understand that the tattooing process involves the use of needles and pigments that may cause discomfort, pain, or allergic reactions.

I acknowledge that there may be potential risks associated with getting a tattoo, including but not limited to infection, scarring, and allergic reactions to pigments or aftercare products.

I agree to follow the aftercare instructions provided by the tattoo artist to promote proper healing and minimize complications.

I confirm that I am not under the influence of drugs or alcohol during the tattooing procedure.

I acknowledge that tattoo results may vary based on my skin type, skin condition, and how well I follow the aftercare instructions.

I understand that tattoos are permanent and removal may require additional procedures, which may not guarantee complete removal or restoration of the skin's original appearance.

I release the tattoo artist, the tattoo studio, and their employees from any liability for any injury, loss, or damage that may occur as a result of the tattooing procedure.

I understand that it is my responsibility to provide accurate medical and personal information to the tattoo artist, including any pre-existing medical conditions, allergies, or medications I am currently taking.

I agree that photographs or videos may be taken before, during, and after the tattooing process for documentation and promotional purposes, with the understanding that my identity will be protected unless I provide explicit written consent.

I agree not to disclose the address or location of the tattoo studio to any third party without the explicit written consent of the tattoo studio.

I acknowledge that my deposit is non-fundable and will be taken out of my final session of the tattoo.
